Many insurances cover the consult, the procedure, and the follow-up visits. You can call you insurance company directly and inquire, or, if you would like us to find out for you, call our office with your information and we will inquire. We set our cash price at the lowest agreed upon reimbursement from the insurance companies we work with. The average cash price is approximately $600.

Step 1: Find Appointment & Pay Deposit

Your $100 Deposit Fee Will Be Processed To Reserve An Appointment Space For You. Please Be Advised This Is A Non-Refundable Deposit Should You Cancel Your Procedure. You May Reschedule One Week in Advance of Your Appointment.

Step 2: Fill out Forms

Please print and fill out the form packet below:

You Must Complete all Forms And Fax Them To Our Office Within 1 Week Of Your Scheduled Procedure Date Or Your Procedure Could Be Postponed.

Step 3: Send in forms

Once You Have Submitted Your Registration, paid deposit, and filled out the forms please send them to our office. We will call you to confirm your appointment on the next business day.
